Résumé

Les manifestations cardiaques de la maladie de Fabry s’intègrent en général dans une atteinte polyviscérale, apparaissent chez l’adulte jeune et sont source de morbi-mortalité élevée. Les complications cardiaques sont présentes chez plus de la moitié des patients atteints de maladie de Fabry. Elles sont accessibles aux traitements habituels de l’hypertension artérielle, de l’insuffisance cardiaque, des coronaropathies, des troubles du rythme ou de la conduction… Le variant cardiaque peut cependant se présenter comme une atteinte cardiaque prédominante, voire isolée, dans laquelle l’hypertrophie ventriculaire gauche est au premier plan, pouvant faire porter par erreur le diagnostic de cardiomyopathie hypertrophique « idiopathique » d’origine sarcomérique (1–4 % des cas, voire 6 % chez l’homme avant 40 ans). Cependant, l’hypertrophie est le plus souvent concentrique et il n’y a en général pas d’obstruction sous-aortique, tandis que la recherche de signes évocateurs de maladie de Fabry (acroparesthésies, anhidrose, dysfonction rénale ou atteinte neurologique) doit être systématique. L’identification précoce des sujets atteints de maladie de Fabry permet la surveillance des organes cibles, l’identification des apparentés, le conseil génétique et enfin le traitement spécifique par enzymothérapie substitutive qui, en l’absence de lésions de fibrose myocardique étendue ou de dysfonction rénale sévère, est efficace sur la progression de la maladie rénale et l’hypertrophie ventriculaire gauche, et réduit parallèlement le risque de survenue d’un événement rénal, cardiaque ou neurologique.

Abstract

Cardiac manifestations of Anderson-Fabry disease are usually part of a multiorgan involvement; they are frequently recognized in the young adult and increase morbidity and mortality. Cardiac complications, first hypertension and diastolic dysfunction, are observed in about half of patients and are accessible to the usual management of hypertension, heart failure, coronary artery disease, rhythm or conduction disturbances. However, the cardiac variant may present as an isolated or predominant cardiac involvement, with left ventricular hypertrophy being the most apparent sign, that could lead to the wrong diagnosis of « idiopathic » hypertrophic cardiomyopathy of sarcomeric origin (in 1–4% of cases, up to 6% in males before 40 years). However, in Fabry disease, hypertrophy is more often concentric without subaortic obstruction, while search for signs of Fabry disease (history of acroparesthesia or anhidrosis, renal dysfunction or stroke) should be systematic. Early identification of subjects with Fabry disease allows to check for target organ damage, family screening, genetic counseling and specific enzyme replacement therapy. The latter, in the absence of irreversible and extended myocardial fibrosis and/or severe renal dysfunction, is efficient on the progression of renal disease and cardiac hypertrophy and delayed in parallel the occurrence of a first renal, cardiac or neurologic event.
