Comparison of impedance cardiography with invasive hemodynamic measurements in patients with heart failure secondary to ischemic or nonischemic cardiomyopathy

It is of paramount importance to characterize the individual hemodynamic response of patients with postural orthostatic tachycardia syndrome (POTS) to select the best therapeutic intervention. Our aim in this study was to describe the hemodynamic changes in 40 patients with POTS during the head-up tilt test and compare them with 48 healthy patients. Hemodynamic parameters were obtained by cardiac bioimpedance. Patients were compared in supine position and after 5, 10, 15, and 20 minutes of orthostatic position. Patients with POTS demonstrated higher heart rate (74 beats per minute [64 to 80] vs 67 [62 to 72], p <0.001) and lower stroke volume (SV) (83.0 ml [72 to 94] vs 90 [79 to 112], p <0.001) at supine position. The response to orthostatic challenge was characterized by a decrease in SV index (SVI) in both groups (ΔSVI in ml/m²: −16 [−25 to −7.] vs −11 [−17 to −6.1], p = NS). Peripheral vascular resistance (PVR) was reduced only in POTS (ΔPVR in dyne.seg/cm⁵:−52 [−279 to 163] vs 326 [58 to 535], p <0.001). According to the best cut-off points obtained using the receiver operating characteristic analysis for the variation of SVI (−15.5%) and PVR index (PVRI) (−5.5%), we observed 4 distinct groups of POTS: 10% presented an increase in both SVI and PVRI after the orthostatic challenge, 35% presented a PVRI decrease with SVI maintenance or increase, 37.5% presented an SVI decrease with PVRI maintenance or elevation, and 17.5% presented a reduction in both variables. Body mass index, ΔSVI, and ΔPVRI were strongly correlated with POTS (area under the curve = 0.86 [95% confidence interval 0.77 to 0.92], p <0.0001). In conclusion, the use of appropriate cut-off points for hemodynamic parameters using bioimpedance cardiography during the head-up tilt test could be a useful strategy to identify the main mechanism involved and to select the best individual therapeutic strategy in POTS.

Blood pressure is a physiologic measure that reflects cardiac output and systemic vascular resistance. Classification by these components could be useful in characterizing subtypes of hypertension, which may have a role in selecting treatment strategies. However, hemodynamic phenotypes of a large, stable, outpatient population with hypertension remain unknown.

We included 34,238 people with systolic blood pressure of ≥130 mm Hg, who underwent impedance cardiography at 51 sites of iKang Health Checkup Centers throughout China between 2012 and 2018. Hemodynamic parameters measured included stroke volume, stroke volume index, heart rate, cardiac output, cardiac index, systemic vascular resistance, and systemic vascular resistance index. We characterized these by systolic blood pressure categories and assessed patient characteristics associated with the ratio of cardiac index to systemic vascular resistance index.

Among the study cohort (n = 33,414; mean age 52 ± 13 years; 36.6% female), 49%, 40%, and 11% had systolic blood pressure130-139, 140-159, and ≥160 mm Hg, respectively. Among patients with systolic blood pressure 140-159 mm Hg, 9353 (70%) had high systemic vascular resistance index but normal/low cardiac index, 1949 (15%) had high cardiac index but low/normal systemic vascular resistance index, and 2053 (15%) had low/normal cardiac index and systemic vascular resistance index. Using multivariable analysis, we found that cardiac index to systemic vascular resistance index ratio was negatively associated with age and body mass index (all P <0.05; R-square 0.16, 0.12, and 0.09 for systolic blood pressure 130-139, 140-159 and ≥160 mm Hg, respectively).

Different hemodynamic blood pressure phenotypes were identified across all hypertensive blood pressure categories. Although individual characteristics were associated with the cardiac index to systemic vascular resistance index ratio, they only weakly explained the variation.

For patients with obstructive sleep apnea hypopnea syndrome (OSAHS), chronic inflammation and hemodynamic oscillations caused by respiratory events contribute to cardiovascular disease (CVD). In this study, a physiological marker named oscillatory coupling factor (OCF) exacted from cardiac output (CO) was introduced. This study aimed to evaluate the clinical value of OCF and tentatively explore its predictive value of cardiovascular prognosis in OSAHS patients.

An impedance cardiogram (ICG) was used to continuously obtain the participants' CO with simultaneous polysomnography. Participants were divided into three groups: an OSAHS–CVD– group (n = 19); an OSAHS + CVD– group (n = 34); and an OSAHS + CVD + group (n = 36). The OCF was exacted from the CO by using empirical mode decompensation-based detrended fluctuation analysis (EMD–DFA).

The OCF values were: OSAHS + CVD + group [1.20 (0.98–1.78)] > OSAHS + CVD– group [1.14 (1.02–1.94)] > OSAHS–CVD– group [0.95 (0.56–1.16)], (p = 0.001). A Spearman test showed that OCF was positively correlated with age, apnea/hypopnea index (AHI), microarousal index (MAI), oxygen desaturation index (ODI), and negatively correlated with the lowest SpO₂. Ten participants were treated by one-night continuous positive airway pressure (CPAP): their AHI decreased from 44.9 (18.0–72.9)/hour to 1.25 (0.0–7.5)/hour, and their OCF fell from 1.17 (1.10–1.69) to 1.08 (0.96–1.23) (p = 0.038). Seventy-seven participants were effectively followed up. Seven participants developed CVD events or newly diagnosed CVD; their OCFs were distributed on a relatively high level [1.18 (1.01–1.56)].

The OSAHS participants had higher OCFs than those without OSAHS, while CVD made the OCFs even higher; CPAP could rectify this change. Oscillatory coupling factor may be a physiological marker of cardiopulmonary coupling and have potential cardiovascular prognostic value for people with OSAHS.
