Although both cyclosporine and tacrolimus induce hypomagnesemia by promoting the downregulation of the main magnesium absorber in the distal tubule, transient receptor potential melastatin 6 (TRPM6), tacrolimus has been shown to decrease magnesium levels to a higher degree compared with cyclosporine [7–11]. In addition, recent data indicate that cyclosporine downregulates epidermal growth factor, an inducer of the magnesium reabsorption via TRPM6 [12].

Additionally, Bolzoni et al.45 reported that multiple myeloma cells are able to inhibit noncanonical Wnt5a, an upregulator of calcineurin, suggesting that myeloma cells may have CNI effects, explaining the lysosomal phenotype in selected patients with light chain disease.
