Summary
Left ventricular dynamics, coronary blood flow (\(\dot V_{cor}\)), and myocardial oxygen consumption (\(M\dot VO_2\)) were determined in normal patients (N), in chronic pressure overload (aortic stenosis, AS), in chronic volume overload (aortic incompetence, AI), and in coronary heart disease (CHD). Peak systolic and enddiastolic wall stress were increased in AS and AI by 26–52 per cent, the systolic stress being preferably increased in AS, whereas enddiastolic stress was markedly greater in AI.\(\dot V_{cor}\) and\(M\dot VO_2\) were elevated in both groups by 44–55 per cent (p<0.001). Sufficient correlation was present between peak systolic wall stress and the\(M\dot VO_2\) (r=0.82). Since at a given wall stress the\(M\dot VO_2\) was somewhat increased in AI and AS when compared with N and CHD, the considerable enhancement of the rate of pressure development (AS) as well as of the external cardiac work (AI) may contribute to the increase in overall oxygen consumption. It is concluded that systolic wall stress represents a primary determinant of\(M\dot VO_2\) in pressure and volume overload and that the\(M\dot VO_2\) increases in these diseases when hypertrophy becomes inappropriate with regard to the pressure and volume demands imposed to the left ventricle.
Zusammenfassung
Ventrikelfunktion, Koronardurchblutung (\(\dot V_{cor}\)) und Sauerstoffverbrauch des linken Ventrikels (\(M\dot VO_2\)) wurden an Patientengruppen mit normaler Ventrikelfunktion (N), mit chronischer Druckbelastung des linken Ventrikels (Aortenstenose, AS), mit chronischer Volumenbelastung (Aorteninsuffizienz, AI) und mit koronarer Herzkrankheit (KHK) vergleichend untersucht. Die maximale systolische Wandspannung war bei Druckhypertrophie (AS) um 51% und bei der Volumenhypertrophie (AI) um 26% über der Norm erhöht. Die enddiastolische Wandspannung war um 52% (AI) bzw. 34% (AS) gesteigert.\(\dot V_{cor}\) und\(M\dot VO_2\) waren in beiden Gruppen signifikant erhöht (p<0,001),\(M\dot VO_2\) zeigte eine befriedigende Korrelation zur maximalen systolischen Wandspannung (r=0.82). Allerdings war die Beziehung zwischen der maximalen systolischen Wandspannung und dem\(M\dot VO_2\) bei AS und AI gegenüber N und KHK bei gegebener Wandspannung zu höherem\(M\dot VO_2\) verlagert, so daß die erhebliche Steigerung der Druckentwicklung bzw. Druckanstiegsgeschwindigkeit (AS) sowie der Schlagarbeit des linken Ventrikels (AI) zum myokardialen Sauerstoffverbrauch meßbar beitragen. Es wird geschlossen, daß die maximale systolische Wandspannung eine vorrangige Determinante des myokardialen Sauerstoffverbrauches bei Druck- und Volumenbelastung darstellt und daß der Sauerstoffverbrauch der druck- und volumenhypertrophierten Herzen dann zunimmt, wenn die systolische Wandspannung als Folge einer im Vergleich zur Druckund Volumenbelastung des linken Ventrikels inadäquaten Hypertrophie ansteigt.
